Agent actions

Review what an agent proposes to do against its grant, limits, approvals and the user’s instruction before your application executes the action.

For teams running AI agents against business systems, before an action executes or after it did.

Where it fits in your workflow

Use this at a tool-call boundary or when reviewing an action log. Send the proposed or executed action, the grant it uses, the user instruction and the relevant input excerpts. This makes the difference between technical permission and the user’s intended task visible in one review.

  1. Capture the action before dispatch. Record the agent and session, timestamp, tool, operation, target resource and arguments. Include whether the action is proposed, blocked or already executed. An integration can use the result in its own dispatch policy.
  2. Check the applicable grant and approval. Provide the grant’s scopes, resource patterns, argument limits, validity dates and revocation time if present. List operations that your policy considers consequential, and include independent approval when required.
  3. Review the instruction and input origin. The meaning of the user’s instruction and the origin of an action are separate judgment questions. Preserve the relevant document, email, web or tool-output excerpts so unresolved intent questions reach a reviewer with their context.

What to send

  • Action. Agent and session IDs, timestamp with an offset, instruction, tool, operation, arguments, grant ID and outcome. Include target_resource when the operation acts on a particular resource.
  • Grant. The grant record for the acting agent: exact tool:operation or tool:* scopes, resource glob patterns, decimal argument limits, issue and expiry times and any revocation time.
  • Session and approval. Relevant actions from the same session for the per-minute rate check; approval identity and time for consequential operations.
  • Inputs and policy. Label excerpts by source: user, tool_output, document, web or email. Supply your consequential-operation list in context.references rather than assuming every sensitive operation is included by default.

Use POST /api/v1/verify for one item or POST /api/v1/audits for a population with shared context. See the request structure.

Allowed by the grant, but not yet justified by the task

The user asks a support agent to summarize inbox messages and add the summary to a ticket. The recorded action instead forwards messages to an address mentioned inside an email. Its grant includes email:forward and allows the inbox resource, so those direct permission checks hold.

The recorded result is uncertain. The instruction-following and input-origin questions are unqualified. In this sample, email:forward is not on the configured consequential-operation list, so the approval check is not applicable. A reviewer should confirm the user’s authorization; an integration that requires approval for forwarding should explicitly include that operation in its policy. The example was already executed, so this verification does not undo it.

Recorded verdict: uncertain. The table below comes from the saved response.

Recorded from the production service on 2026-10-03T04:20:13Z. It was recorded with agent-actions@1; a request naming agent-actions now runs agent-actions@2, so its test labels and configuration differ. The use-case page lists what changed between versions. This page displays a saved example, not a new verification request. Sensitive identifiers are masked in public examples.

Supported records and checks

These definitions are generated from the registered use-case pack. A procedure computes a result from the supplied data; a question can decide a check only when it is qualified for the model and meets its thresholds. The descriptions name the checks; they do not authenticate the underlying business events. Supplied fields, defaults and comparison populations determine what each procedure can establish. Risk weights help prioritize review and are not probabilities.

  • action: One action an AI agent took or proposed: a tool call with its arguments, under a grant, on a user's instruction.

Judgment questions and qualification

The table describes published evaluation evidence. For the qualification attached to the running service, inspect this use case’s live definition. A model answer can be recorded without deciding a check. How qualification works.

Scope and limits

  • OpenAuditor returns a finding. Your application must call it at the right point and enforce the resulting allow, block or review policy; the API does not intercept or roll back tool calls itself.
  • Grant checks are exact comparisons against the supplied data. Resource globs, operation names and argument limits must describe the operation you actually dispatch.
  • The current recorded example does not demonstrate qualified prompt-injection detection. Unresolved intent and origin checks remain visible even when every applicable permission check holds.

A true verdict means all applicable, unskipped checks hold within the decision gates; false means at least one qualifying failure decided the item. Other assertions can remain unresolved even with a false verdict. An uncertain verdict means the evidence and decision gates do not support a decided result. Review the assertion outcomes and evidence before taking a business action. Read the decision method.

Published evaluation results

These measurements use synthetic populations with generator-stated truth. Coverage is the share receiving true or false verdicts, not measured accuracy on your records. Read the metrics and their limits.

Versions

A request naming agent-actions runs the current version. Name a version, such as agent-actions@1, to keep the checks you integrated against; a superseded version stays served unchanged. Test labels in a response carry the version that ran them.

VersionWhat it checks differently
agent-actions@2The current version, used when a request names agent-actions alone. An action without a target_resource is checked against its grant first: a missing grant is insufficient_evidence, as for every grant check, and so is a found one, since a grant's resource patterns allow only what they match (an empty list allows nothing) and an unnamed resource cannot be shown to be allowed. Every other check is version 1's.
agent-actions@1Superseded, and still served unchanged to requests that name agent-actions@1. resource_allowed is not applicable to any action without a target_resource, whatever its grant allows.

Schemas and integration

Every item, record and reference schema is at GET /api/v1/use-cases/agent-actions.

Supported context record types: grant, action.

Supported reference sets: consequential_operations, calendar.

Start with a reviewable sample

Start in observation mode on a small set of consequential tools. Compare proposed calls with grants and human review, define how uncertain results are handled, and then place the check before execution where your workflow requires it.
